FORT LAUDERDALE —A drug that has been widely adopted as a new treatment for men with impotence, Viagra has been nicknamed "the little blue pill" and "the little blue pill of impotence" for years. In an attempt to keep Viagra affordable, the U. S. Food and Drug Administration has approved the little blue pill for marketing.
The drug, called a phosphodiesterase type 5 (PDE5) inhibitor, is taken orally, not by mouth. Instead, it is injected into the muscle in a tiny vein and is given to the man for five to ten days, then continued, with no more than a few minutes between injections. It works by increasing blood flow to the penis, which is the opening of the blood vessels supplying the penis. It is approved by the FDA for use in men with impotence, a condition in which the penis is hard to close. It is also approved for use by men who have not previously had trouble with erections, such as those with low blood pressure.
Although it was originally intended for people who had lost a few pounds, a Food and Drug Administration review found that Pfizer Inc. and its partner Eli Lilly & Co. also approved the drug. The FDA also reviewed the marketing authorizations of about 6 other drugs that are now marketed under different names such as Viagra, Cialis, Levitra and the new drug class.
The drug's marketing authorizations are also based on studies in men with a history of heart disease, high blood pressure, and a history of a low blood pressure. The drugs also use a marketing technique called PDE5 inhibitor and are approved only for use in men over the age of 18.
In terms of its effects, the new drug has a "strongest effect" on a man suffering from impotence, said Dr. David H. Hahn, a Harvard Medical School professor of medicine.
"This is a medication that has been in use for many years and has been shown to work as well as it had been," Hahn said. "I have seen it work as well as any other drug. But there are also those people who need it more than I do. They have a history of heart disease, high blood pressure and diabetes. They are very different from me."
Hahn said the new drug may be an option for some men with a history of heart disease.
Hahn said he also was pleased to see that Viagra had been approved for use in men who had recently had trouble with erections, and had had a low risk of developing heart disease. He said that the medication is also being used as an effective alternative for some men who have diabetes and who take nitrates. Hahn said Viagra is not a cure for erectile dysfunction, but it will be a treatment for patients who cannot take them.
While Hahn said there is no evidence that the new drug will work for men with heart disease, Viagra has been shown to have a "strongest effect" on the heart. It also has been shown to improve blood flow to the penis, which makes it easier to get and maintain an erection. Viagra is available by prescription only.
There are currently no studies that show whether Viagra will work in men with heart disease, and Hahn said that Viagra is not an option for men with these conditions. He said it is important to have regular follow-up with the doctor to monitor for any adverse effects.
In the meantime, Hahn said, men who have low blood pressure and diabetes may want to consider using Viagra. He said the new drug will help men who are suffering from heart disease, have diabetes and have lost their ability to exercise. "It is an important tool in the battle against impotence," he said.
